Hydrostatic test fitting fight cost me 6 hours

Had a flange gasket keep blowing out at the weld neck on a 4 inch line near Gary, Indiana. Took me all morning to realize the studs were torqued in the wrong sequence, not the gasket quality. Anyone else ever burn a shift on something that basic?

Torqued in the wrong sequence got me too, spent a whole day chasing a leak that was just crisscross order.
