Appreciation post: That old boilerplate in the ASME code got me thinking

Was reading through Section VIII of the ASME code last night. Found a stat that blew my mind. Did you know that over 60% of boiler failures happen from operator error, not manufacturing defects? That's wild when you think about all the time we spend on weld inspections and material testing. But then again, I've seen guys cut corners on water treatment and blame the boiler. So is it really operator error, or is it just poor training? What's your take on where the real blame lies?

Haha, I've definitely been THAT operator who couldn't figure out why the pressure was dropping on a Monday morning.

I read that Monday mornings are always rough for catching pressure issues.
